Visa Screen Application for RNs

I've read many posters from different forums asking how to apply for the visa screen. This is the reason why I came up with this idea to help you guys out.

You need the following requirements:

  • A bank check or international money order (drawn on US banks in US funds) made payable to ICHP; or credit card (Visa, MasterCard or Discover), for the full application fee in US dollars within 60 days of the application date. DO NOT SEND CASH.
  • Documentation of your secondary school (High School) Education, with English translations.
  • "Academic Records form" and transcripts from each of your profesisonal schools. (Page 18)
  • "Request for Validation of Registration/License" form from each licensing agency where you have ever held registration/license/certification as a professional in your field. (page 17)
  • "Verification of License" from your state of BON or evidence of passing the CGFNS Qualifying exam. Page 17)
  • 2 passport-sized photographs, signed on the front and the back.
  • Certified English translation of all documents not in English.
  • English Language Proficiency Assessment

Note: If you applied for the CGFNS qualifying exam, no need to send again your academic records, PRC Verification and photographs.
